PATIENT-CENTERED MEDICAL HOME
Old Hook Medical Associates is now nationally recognized as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA)
A PCMH emphasizes enhanced patient care through open scheduling, expanded hours and strong communication among patients, physicians, and staff. This system will lead to better health care quality, improved involvement of patients in their own care, and a reduction of costs. The PCMH structure integrates seamlessly with the Old Hook Medical electronic medical records system and Patient Portal that allows physicians in the practice to have access to records 24 hours a day and permits patients to securely review their medications, immunizations and other health information online.
Being a PCMH guarantees each patient will have an ongoing relationship with a personal physician who leads a team providing for all the patient’s health care needs. When appropriate, this team leader physician will also arrange for needed care with other health care professionals both inside and outside of Old Hook Medical.
Old Hook Medical firmly believes in establishing practices such as PCMH that help deliver superior care while staying firmly rooted in medical evidence.
